Please be aware of the following

  • A consent to release information form signed by provider is required before any information can be provided to a third-party requesting claim history and coverage verification.
  • Claims History responses may take up to two business days upon receipt of request.
  • This portal is not to communicate with the Medical Staff Department.

Insurance Information for all UNC Health Southeastern Employed Medical Providers:

UNC Health Southeastern provides professional liability insurance coverage for its employed medical providers through a Self-Insured Trust, administered by the UNC Health Southeastern Risk Management Department.

Coverage is provided on a claims-made basis and includes an extended reporting endorsement (Tail Coverage). Coverage is limited to medical and professional healthcare services performed on behalf of UNC Health Southeastern during the coverage period.

Risk Management will report claims or litigation arising from alleged professional liability in which the provider was either a named party, or in which the provider's care was the subject of the claim and reported to us within the past six (6) years. We will also report with respect to claims, which has not been the subject of suit, if settled by payment on behalf of the provider at UNC Health Southeastern.
