What we do:
- Screen patients for eligibility
- Complete the entire PAP application on your behalf
- Coordinate with your healthcare provider
- Provide free medications once approved
What patients need to bring:
- Proof of income
- Proof of residency
- Photo ID
- Medicaid denial or Family Planning letter (if applicable)
- Current prescriptions (hard copy)
- Medicare patients: Low-Income Subsidy application

There is NO COST for this service or for medications
received through the program.

Safe, convenient management for patients taking warfarin (Coumadin).
Services:
- INR testing with point-of-care equipment
- Warfarin dose adjustments to help achieve and maintain a therapeutic INR
- Education on diet, medications, and lifestyle factors that may affect INR
- Counseling on signs of bleeding or clotting
- Coordination with your healthcare provider for ongoing management
