Patient Assistance Program (PAP) Support

Anticoagulation Clinic

Free medication assistance for patients who qualify.

What we do:

  • Screen patients for eligibility
  • Complete the entire PAP application on your behalf
  • Coordinate with your healthcare provider
  • Provide free medications once approved

What patients need to bring:

  • Proof of income
  • Proof of residency
  • Photo ID
  • Medicaid denial or Family Planning letter (if applicable)
  • Current prescriptions (hard copy)
  • Medicare patients: Low-Income Subsidy application
    (our staff can help complete this in office or by phone)

There is NO COST for this service or for medications
received through the program.

Safe, convenient management for patients taking warfarin (Coumadin).

Services:

  • INR testing with point-of-care equipment
  • Warfarin dose adjustments to help achieve and maintain a therapeutic INR
  • Education on diet, medications, and lifestyle factors that may affect INR
  • Counseling on signs of bleeding or clotting
  • Coordination with your healthcare provider for ongoing management
